New Patient Registration
The surgery covers Moulsecoomb, Bevendean, Coldean, Falmer, Hollingdean and the area of Brighton north of Elm Grove. You may register with the Practice only if you live within these areas.
In order to register you will need to be eligible for NHS services and live within the above areas. You will need to provide two forms of identification one of which must include your current address and one must be photographic ID.
Medical treatment is available from the date of registration. Please contact reception for further information.
You can may print off a GMS1 registration form and The Avenue Surgery's Adult New Patient Registration Questionnaire (links below), fill them out and bring them with you on your first visit to the practice. If you wish to register a child or register for online services you will need to pick up the Registration Form from Reception.
